The current focus on antidepressants for the treatment of anxiety may have been somewhat overstated. A recent survey by the American Psychiatric Association (APA) found that, in many patients with anxiety, antidepressants are the first-line treatment for a variety of symptoms, not treatment for the specific symptoms of anxiety. However, the data on first-line antidepressant use are limited, and some patients with anxiety may be better advised to switch to another treatment. Specifically, patients with posttraumatic stress, bipolar (manic) depression, generalized anxiety disorder (GAD), obsessive-compulsive disorder (OCD), and related disorders, were most likely to switch from their initial antidepressants to another antidepressant. The data also found that those who received their first antidepressant in the first six months of treatment (not those who started treatment later) were the least likely to switch; however, those who received their first antidepressant in the last 6 months of treatment were the most likely to switch. Patients who discontinued a first antidepressant within the last 6 months of treatment, however, had a greater risk of switching to another antidepressant.
This review summarizes the key research on the efficacy of antidepressants as first-line treatments for anxiety and mood disorders. These antidepressants are typically well tolerated, have short-lived effects, have few adverse effects, and generally produce short-lived rebound anxiety after initial treatment ends. This suggests that even patients who previously have not been diagnosed with a diagnosable disorder may benefit from a combination of antidepressants, if they receive adequate treatment followed by active treatment.
